Where does state live?

In your DynamoDB, in your account. The gateway is stateless and rebuilds from that state.

Can we run it without public access?

The deployment can sit in private subnets. Reaching the endpoint is your networking choice, not a requirement of the product.

Is the source auditable?

Yes. The control plane is MIT licensed and the deployment is plain OpenTofu, so reviewers can read every line.
